Position: Case Worker
Duty station: Brak Alshati – Libya
Report to: Senior Case Worker
Status: Full time
Main Purpose
Under the guidance of the Senior Case Worker, the case worker will be responsible to identify and assess cases in need of individual support. This includes opening of cases and prepare all documentation; individual support through planning for response and care; referral and liaising with support services when required; monitoring, follow up and review all the cases. |
Tasks
- Identify individual cases through regular presence in the community and accept referrals internally or from other agencies and community partners.
- Conduct initial (rapid) assessments for individual cases and prioritize them according to risk level.
- Develop case plans that respond to the needs addressed in initial and comprehensive assessments and seek support of supervisor when necessary.
- Regularly follow up to ensure all services and action points listed in the case plan are carried out within agreed time frames. Ensure that progress is regularly reviewed.
- Regularly monitor and support individuals of concerns and their families through home visits, providing guidance, advice and emotional support, community mediation and referrals.
- Work with supervisors and managers to arrange case meetings for complex cases and ensure children receive multidisciplinary support.
- Manage cases in line with existing SOPs, adhere to standard documentation processes and follow best practice guidance.
- Regularly document cases using case notes and other agreed upon forms, update databases to ensure a comprehensive record of the case.
- Ensure that data collection and storage respect data protection protocols and confidentiality principle.
- S/he will perform any other tasks requested by the Supervisor in the framework of the activities foreseen by INTERSOS program in Libya.
